All flexible benefit plan participants should have received a mailing this week regarding Open Enrollment, which runs through 5:00 PM on Tuesday, September 29th. The link to Open Enrollment materials, including Self Service instructions to complete enrollment on-line, is as follows:
This is the one time each year when all plan participants can change medical, dental and life insurance elections. Please make sure that if you are making a change that you complete all of the appropriate forms by September 29th. This applies if you are enrolling for medical coverage for the first time, if you are switching from Personal Choice to Keystone and need to select a primary care physician, if you are adding new dependents to any medical or dental plan, or if you need to have medical underwriting performed due to an increase in voluntary life insurance. Human Resources will have all of the forms at the Benefits Fair, which is on Tuesday, September 15th from 11:00 AM – 2:00 PM in the Thomas Great Hall. The attached flyer indicates all of the vendors who will be attending. As always, the vendors come with give-aways and several have prize and gift card raffles.
The high deductible health plan (HDHP)) became even more affordable with the current renewal. Representatives from Independence Blue Cross and Armstrong Doyle & Carroll will be at the Benefits Fair and will be available to answer any questions that you may have on the HDHP. There will also be an HDHP informational session onMonday, September 21st, from 10:00 – 11:00 AM, in the Gateway Conference Room.
There are slots available for the blood glucose and cholesterol readings being held at the Benefits Fair by AREUFIT. Please contact Deborah Harley at ext. 5261 if would like to reserve a time slot. This is a “non-fasting” blood test. There will also be a nutritionist at the Benefits Fair, who is available to answer questions on “first-come, first-served” basis.
I am also using Open Enrollment as a reminder that employees need to stay current with emergency contact and insurance beneficiary designations. I would encourage everyone, particularly longer-term employees, to take the time to review this information. Emergency contacts can be updated in Bionic Self Service. The form for changing life insurance beneficiaries is:
Completed life insurance beneficiary forms will be collected at the Benefits Fair. The policy numbers requested on the form are FLX 964277 for basic coverage and FLX 964280 for voluntary coverage.
Beneficiary information should be kept current with TIAA-CREF ( www.tiaa-cref.org ) and Vanguard (www.vanguard.com ) by entering this data on-line. If you log into TIAA-CREF’s site, you should look for the “I Want To…” box on the bottom right of the home page and select “Update My Beneficiaries”. On Vanguard’s site, you need to go to the “My Profile” tab and select “Beneficiaries:”. Both Vanguard and TIAA-CREF will be at the Benefits Fair.
